Truth Shepard Natural Health Association (“TRUTHSHEPARD” or “we,” “us” or “our”) recognizes that your privacy is very important and we take it seriously. This Privacy Policy governs the manner in which TRUTHSHEPARD collects, uses, processes, maintains, and discloses information collected from users (each, a “User”) of the https://truthshepard.com website (“Site”). TruthShepard.com is the website for the Private Membership Assiociation (PMA) for Ruth Shepard, CHom, CST, HP Supervisor, Yoga Instructor and Shamanic Practitioner. It also describes your choices regarding the collection, use, and processing of your information. This Privacy Policy applies to the Site and all products and services offered by TRUTHSHEPARD, including any emailed newsletters, other interactive features and communications however accessed and/or used that are operated by TRUTHSHEPARD, made available by TRUTHSHEPARD, or produced and maintained by TRUTHSHEPARD, and any other website, application, service, or tool on or in which this Privacy Policy is posted, linked, or referenced (collectively, the “Services”).

The Services are owned and operated by TRUTHSHEPARD and its affiliates.

Please note that this Privacy Policy does not apply to information collected through third-party websites that you may access through the Services.

USER CONSENT AND CHANGES TO THE PRIVACY POLICY

PLEASE REVIEW THIS PRIVACY POLICY CAREULLY. By using the Services, you accept the terms of this Privacy Policy and consent to our collection, use, processing, disclosure, and retention of your information as described in this Privacy Policy. For example, our third-party payment processor collects and shares information with us in connection with charitable donations to TRUTHSHEPARD through the Services.

INFORMATION FROM CHILDREN

As a participant in clinics and workshops I, herby, grant my permission to TruthShepard Natural Health Association and graduates of  TruthShepard’s advanced trainings the rights of my and/or my child(ren)’s image, in video or still, and of the likeness and sound of my and/or my child(ren)’s voice as recorded on audio or video without payment or any other consideration. I understand that my and/or my child(ren)’s image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my and/or my child(ren)’s likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my and/or my child(ren)’s  image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to:

  • Presentations;
  • Courses;
  • Online/Internet/Website;

By signing up for a clinic or workshop with TruthShepard Natural Health Association, I understand that I am agreeing to this release. Statement of Consent for TruthShepard Natural Health Association

I agree to the following statement: I am a legal adult and have voluntarily requested services from Ruth Shepard Trode, CHom, CEASE Therapist, and Homeoprophylaxis Supervisor, for myself or my child(ren)/ward(s). This office offers teleconference appointments and the contact number is (612) 616-0310, or TruthShepard.com. I assume all responsibility for administering any homeopathic treatment or supplements to myself and/or my children or ward(s) appropriate to my own or my child’s current health needs.

Ruth Shepard Trode has been designated a Certified Homeopath by Homeopathy School International (2015) and a CEASE Therapist from Tinus Smits , M.D., training (2017) and received Certification in Homeoprophylaxis (C.HP) from Free and Healthy Children International, and Isaac Golden and Cilla Whatcott in 2015. Ruth also regularly pursues additional professional training in specialty areas such as Women’s Health, Children’s Health and Travel Homeoprophylaxis. Ruth Shepard Trode also has certification as CranioSacral Therapist, Childbirth Educator, Hypnobirthing and Birth Doula.

I have read and understood the information in this Statement of Consent and understand that Ruth Shepard Trode is not a licensed health care provider, as she does not practice in an area of health care regulated by the State of Colorado. I understand her training is specific to homeopathy and Craniosacral Therapy. I understand that Ruth Shepard Trode and TruthShepard.com, PMA does not carry malpractice insurance.

If I wish to discontinue any medications that have been prescribed for me or my child, I will do so under the supervision of a medical doctor trained in the use of the kinds of medication being taken. If I have a medical complaint or wish to receive a diagnosis of my own condition or that of my child(ren) or ward(s), I will consult with a board certified physician or medical doctor. I further understand that Ruth Shepard Trode does not diagnose, treat, prevent or prescribe for any disease, syndrome or condition but will be working with me or my child(ren)/ward(s) to help me/my child(ren) move toward better health by improved vitality.

Although few medical doctors are trained in the tenets and application of Homeopathy or Craniosacral Therapy, I understand that I should discuss any recommendations made by complementary and alternative health care practitioners with my primary care physician, obstetrician, gynecologist, oncologist, cardiologist, pediatrician or other board certified physician.

If the person being seen by Ruth Shepard Trode is a child, I represent that I have authority to make healthcare decisions for myself or my child/ward, and in my judgment it is in this person’s best interest to receive homeopathic care, and that if I have any concerns or questions whatsoever about this person’s health, I will take my child to a competent physician in a timely manner. I understand that everything in this Statement of Consent that refers to me or the client, also refers to my child or ward, as appropriate.
